In addition, there are various questionnaires available here, which you must fill in and bring to your exam. These forms are mainly concerned with potential contrast agent incompatibilities as well as any pre-existing conditions that need to be considered.
Please complete the questionnaires as far as possible. In case of questions or ambiguities, we will of course help you on the day of your exam.
